Thanks for clicking on this link to complete this survey written by a care leaver for care leavers to create a new play telling the stories of care leavers.
There are only a handful of required questions - and where it is an open space, please write in as much detail as you feel comfortable.
After I have received a number of responses, I will collate the information and start creating real life stories from the responses from this survey. Once the stories and the play has been written, I aim to stage it using care leavers as the actors, directed by a care leaver, care leavers as the backstage crew and produced by a care leaver.
A couple of confidentially things: Firstly, please don’t use anyones names unless you are bigging up or praising someone involved in your care. Secondly, everything you put here is confidential and anonymous - if at any point you wish to retract your answers, get in touch through the methods below and I will remove you from my file.
Finally, if you would like to tell me your story in more detail or wish to be a part of the performance process - get in touch via the methods below. It would be amazing if each story was told by the person who’s story it was. I feel that these stories could have a real impact on the social care system.
